Key Components of the Service

1. Client Needs Assessment

  • Initial Consultation (online, phone, in-person)

  • Assess needs such as:

    • Type of care (dementia, nursing, palliative)

    • Desired location

    • Budget, funding options (private, CHC, LA)

    • Cultural/language preferences

    • Availability of family or support networks

2. Care Provider Matching

  • We Maintain a curated database of CQC-registered care homes with up-to-date information.

  • Match individuals to suitable homes based on:

    • Availability

    • Specialisms

    • Inspection ratings

    • Staff-to-resident ratio

    • Amenities and cultural fit

3. Coordination and Support

  • Arrange tours, visits to short-listed homes.

  • Liaise with social workers, GPs, discharge teams, and funding bodies (e.g., CHC, local authorities).

  • Support application and transition process (e.g., documentation, assessments).

4. Information and Advocacy

  • Offer information about:

    • Types of care and funding

    • Power of attorney, safeguarding, and consent

  • Act as an advocate for the family during decision-making.

5. Post-Move Support (Optional Add-on)

  • Follow-up with the family and care provider to ensure satisfaction and appropriate settling-in.

  • Offer ongoing case monitoring or re-matching if care needs change.
